All student athletes will need to complete the Activities Eligibility Form and the Bellin HIPPA and Consent to Treat forms to participate. To find these forms, please visit the Forms page on the District website.

Online Athletics Registration
Online Registration Steps:
- Go to Franklin’s registration page.
- If you have previously registered online, choose Login to My Account, otherwise, continue with the steps below.
- Click on "I don't have an account".
- Enter first and last name. Select a username and password that you will remember. Enter a valid email address. Check the box I’m not a robot. Then click Sign Up
- Check your email and confirm your account setup. Once you have verified your account, return to the online registration site and log in.
- Click the Register link and select "Franklin Athletic Registration Form 2024-2025".
- Enter your student information. Then click Next Page.
- Select the sports the student plans to participate in and click Next Page.
- Enter the parent/guardian information and click Next Page.
- Enter medical information and emergency contacts as well as Student Emergency Information. Check the appropriate boxes and sign electronically.
- Review the Concussion and Sudden Cardiac Arrest Information and check the boxes.
- Review the Co-curricular code information and check the boxes.
- Sign electronically and click Next.
- Click the Submit button on the Family Account Login page.
- Administrators will be notified that you have submitted your registration. If it is incomplete, someone will contact you via email.
Franklin Football is gearing up to start our 2024 season. A parent meeting will be held on Thursday, August 8 at 5:30p in the Bistro at Franklin Middle School. Our first practice is Monday, August 19 at 3:00p. We are looking forward to starting our season.
